12/9/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C arrived to the home of Laylah. Upon arrival to the home setting, Laylah appeared to be in a content mood evident by her facial expressions such as a smile on the face. QMHP-C informed family this session was going to consist of a counseling held with Laylah Ms. Boone in the home. QMHP-C facilitated a family discussion with Laylah and her Ms. Boone. QMHP-C engaged both Ms. Boone and Laylah in a discussion to determine any new behaviors since the last session. Ms. Boone discussed with QMHP-C Laylah continues to present the same behaviors such as anger issues, communicating and just talking back to her (mom) with behaviors lasting 2-3 minutes in duration. 2. Behavior: Laylah participated in the session and did not need any redirection from QMHP-C during the session. Laylah presented normal behaviors throughout the session and continued to fail to discuss her challenges associated with anger and communication and when she cannot have her way she fails to communicate with others throwing item with behaviors lasting 5 minutes in duration. 1. Intervention: QMHP-C assessed each otherâs mood by observing communication tone and body language. QMHP-C instructed each of them to elaborate on their feelings. QMHP-C validated responses regarding feelings. QMHP-C role played situations relevant to respecting others with the family and allowed each of them to share feedback regarding the role play. The mother was assisted with parenting skills. Ms. Boone and Laylah were instructed to review roles of each person in the home. Ms. Boone and Laylah was assisted with setting limits and the appropriate way to maintain limits. Laylah and her mother was engaged in a potentially challenging situations and Ms. Boone were observed as they reacted to how Laylah responded to the challenging situation. 2. Intervention: QMHP-C provided support for Ms. Boone throughout the session. QMHP-C emphasized the importance of resolving conflicts in a peaceful manner. Ms. Boone and Laylah was given a situation and they were instructed to respond in such a way that the conflict could be resolved peacefully. QMHP-C provided them each with more alternative ways to respond to the situation. Ms. Boone was educated on how to assist Laylah make better choices in the home, school and in the community. QMHP-C utilized past situations to guide the Ms. Boone with her assistance to Laylah as it relates to making choices. 1. Response: Laylah and her family actively participated in this session. Laylah and her mother had limited feedback amongst each other. Laylah appeared to be somewhat easily agitated during this session. She appeared to have wanted all of the attention focused on her as evidenced by her constantly blurting in when others were speaking. 2. Response: At times, she appeared to be disconnected during the session. The family has difficulty establishing and maintaining roles in the home consistently. The family struggles to resolve conflicts peacefully but has noticeably taken more appropriate steps to do so. QMHP-C Interpretation of clientâs progress towards todayâs objectives: Laylah continues to struggle with being able to resolve conflict within the home, school, and community. QMHP-C Plan of Action: VACCD staff will continue to focus on problematic behaviors that are outlined in her service plan.
